“…According to a consensus on melanonychia nail plate dermatoscopy, it is always necessary to rely on clinical suspicion (history and physical examination) when deciding to perform a nail biopsy rather than dermoscopic pattern alone (Di Chiacchio et al, 2013a, Di Chiacchio et al, 2013b). On the other hand, intraoperative dermatoscopy had a higher sensitivity and specificity (Di Chiacchio et al, 2010, Göktay et al, 2015, Hirata et al, 2011). …”

“…The entire lesion, when possible, or the most irregular pattern area in the nail matrix seen by polarized light dermatoscope (Dermlite; 3Gen Inc, San Juan Capistrano, CA), was chosen for biopsy. [16][17][18] Surgical Technique…”

“…The other 125 refused to undergo an invasive procedure. The average age among the biopsied patients was 45.5 years (19-74 years), and 45 were women (77.6%). Most of the analyzed patients were classified as skin Type V (29, 50%), followed by skin Types VI(16, 27.5%), and IV(13,22.4%) patients.Melanonychia with brown coloration of the background was predominant among the biopsied patients (41, 70.7%). A black background was observed in 17 (29.3%) patients.…”

“…The reliability of dermoscopy in the diagnosis of melanoma is particularly high in the presence of the micro-Hutchinson sign. Dermoscopy may be used to look at the pattern of pigmentation of the nail matrix [36], for which it has a high sensitivity and specificity. With an intraoperative method, a Brazilian group identified 4 dermoscopic patterns: (1) a regular gray pattern, typical of hypermelanosis; (2) a regular brown pattern, typical of benign melanocytic hyperplasia; (3) a regular brown pattern with globules or blotches, typical of melanocytic nevi; and (4) an irregular pattern, typical of melanoma.…”
