PCOS and Role of Cumulus Gene Expression in Assessing Oocytes Quality

Abstract: The global infertility rate has been declining from year to year. PCOS is one of the treatable accountable causes contributing to anovulatory infertility. Nevertheless, the success rate of treatments and live-birth outcomes especially involving assisted reproductive techniques is still not very promising. There is a reduction in the development potential of oocytes and high-quality embryos in PCOS patients compared to non-PCOS patients. A critical step in IVF treatment is the assessment of oocyte and embryo co… Show more

“…Complex biological processes, such as those involved in human reproduction, require different genes working in an orchestrated way to ensure regulatory networks ( de la Fuente, 2010 ). Recent studies of global gene expression in oocytes and sperm have demonstrated important target genes involved in conditions affecting human reproduction ( Hermann et al, 2018 ; Li et al, 2021 ; Ntostis et al, 2021 ; Santiago et al, 2021 ; Sayutti et al, 2022 ). In the present study, prior filtering of meiosis-related genes was performed, and these genes had their differential gene expression evaluated in female and male gonads and gametes under physiological and pathological conditions.…”

“…Of note, a good balance of cumulus growth and apoptosis is required for optimal ovulation. Cumulus cell apoptosis causes poor oocyte results, embryo fragmentation, and impaired blastocyst development [59,61]. These results suggest that the low concentration of fructose would produce a good quality COCs that the higher ones due to the effects of the higher concentration of fructose alters the expression of apoptotic genes and HIF-1α.Other studies proved that women who were successful in becoming pregnant had a considerably lower percentage of apoptotic cells than those who were unsuccessful [62].…”

“…In women, the small dosage supplementation of fructose to the in-vitro maturation media suggests positive expression of HAS2, PTX3, GREM1, and VCAN that correlated with good quality oocytes and can be used as an indicator among PCOS women. These genes are responsible for cumulus expansion, cumulus cell apoptosis, and glucose metabolism [59]. The higher dose of fructose was reported to reproduce some of the features of metabolic syndrome, most changes were caused by oxidative stress and insulin resistance [54].…”

“…This syndrome produces side effects related to women's overall health, such as abnormal hair growth, weight gain, irregular menstrual cycle, infertility and depression [2,3]. Almost 70% of women with PCOS suffer from infertility [4,5], and this condition is one of the common reasons for female infertility [6,7]. PCOS occurs in women during their reproductive years [8][9][10], between 18 and 45 years old.…”
