Pectoralis Major Rupture: Evaluation and Management

Abstract: Pectoralis major tendon ruptures are being reported with increasing frequency and primarily occur in young, high demand, male patients. The injury results from an eccentric contracture of the muscle most commonly while performing the bench press maneuver during weight training. In the setting of both acute and chronic injury, physical examination is critical for a timely and accurate diagnosis. During physical examination, comparison with the opposite side is imperative, and findings may include swelling, ecch… Show more

“…PM tears have been reported anywhere between the muscular origin and insertion, but by far, the most common site of rupture is either the musculotendinous junction or the site of humeral insertion. Rarely, however, do they occur in the muscle belly itself, and when they do, it is primarily due to blunt trauma, not repetitive throwing as witnessed in this case 1,6,7,10,14,19,20 . Intramuscular repair presents a unique challenge to surgeons given the lack of reliable suture constructs and increased likelihood of suture pullout, making an adequate end-to-end repair difficult 21,22 .…”

“…The PM muscle is a large muscle responsible for adduction, internal rotation, and flexion of the humerus and is divided into a clavicular and sternal head with the inferior fibers of the sternal head being the shortest in length 14,15 . Owing to the varying lengths of the PM muscle fibers, the inferior fibers are lengthened to a greater degree relative to their resting fiber length as compared with the more superior fibers when the humerus is extended.…”

“…The sternocostal head is innervated by the medial pectoral nerve, which stems from the medial cord. The PM receives arterial supply from the pectoral branch of the thoracoacromial artery and drains through the pectoral vein into the subclavian vein 1 .…”

“…Rupture of the pectoralis major muscle is an uncommon injury that occurs| more frequently in adults between 20 and 40 years old. 1 The main causes of pectoralis major muscle rupture include weight lifting (particularly bench press), followed by trauma resulting from football, water skiing, wrestling, and ice hockey. 2 Although this injury predominantly affects male individuals, women have been gradually increasing their participation in these predisposing sports.…”
