Pediatric Extracorporeal Life Support Organization Registry International Report 2016

Abstract: The purpose of this report is to describe the international growth, outcomes, complications and technology used in pediatric extracorporeal life support (ECLS) from 2009 to 2015 as reported by participating centers in the Extracorporeal Life Support Organization (ELSO). To date, there are 59,969 children who have received ECLS in the ELSO Registry; among those, 21,907 received ECLS since 2009 with an overall survival to hospital discharge rate of 61%. In 2009, 2,409 ECLS cases were performed at 157 centers. By… Show more

“…The most common bleeding complications during ECMO include intracranial hemorrhage (ICH), surgical site bleeding, and gastrointestinal hemorrhage ( Table 1). Surgical site bleeding is the most frequent complication and occurs in up to 25% of pediatric cardiac and respiratory ECMO patients [1]. Bleeding events are associated with increased mortality [2].…”

“…Anticoagulation during extracorporeal membrane oxygenation (ECMO) is necessary to prevent catastrophic circuit clotting, but it contributes to significant morbidity and mortality. The Extracorporeal Life Support Organization (ELSO) international registry shows a 24% increase in the number of patients placed on ECMO and a 55% growth of centers utilizing ECMO from 2009 to 2015 [1]. Although there has been rapid growth in ECMO around the world, pediatric mortality rates have remained static or even increased depending on the reason for cannulation [1].…”

“…ECLS is supportive therapy utilized when traditional therapies have failed, allowing patients time to heal from injury or inflammation, or alternatively providing support until a definitive treatment can be applied. The 2016 Extracorporeal Life Support Organization (ELSO) registry report records that neonates (defined as patients aged 28 days or less at the time of ECLS initiation) comprise the largest group of patients < 18 years of age requiring ECLS . These critically ill neonates have specific nutrition needs and require significant, early, and aggressive nutrition support prescribed by knowledgeable providers.…”

Furthermore, the presence of malnutrition and underweight status is associated with increased mortality in children receiving ECLS.…”
