The occurrence of keratinous cyst in the oral cavity is extremely rare. Keratinous cyst in the floor of the mouth is painless, doughy or fluctuant lesion and causes no symptoms until it is large enough to interfere with speech or eating. Cyst generally presents as slow and progressive growth and often not diagnosed until the second or third decade of life.We report an uncommon presentation of keratinous cyst of the sublingual space in a 6year old female child. The swelling was mimicking ranula on inspection. On complete examination, the swelling appeared as epidermoid cyst. After a routine evaluation of the sublingual mass, surgical excision was performed under general anaesthesia. This case report addresses novel information regarding simple yet effective way of managing a sublingual keratinous cyst in a paediatric patient.

Los quistes epidermoides son lesiones quísticas benignas que se desarrollan de componentes epiteliales anormales de tejido ectodérmico formado durante el periodo fetal (congénito) o por epitelio implantado después de una cirugía o trauma (adquirido). El quiste epidermoide es considerado una lesión benigna que afecta la región del pericráneo, cara, cuello, espalda y torso, el 7% de estos quistes ocurren en la región de la cabeza y el cuello y solo el 1,6% representa la cavidad oral. A continuación, se presenta un caso clínico con diagnóstico histopatológico de quiste epidermoide en la región parotídea, el cual no presentó un compromiso del parénquima glandular, el cual fue tratado mediante escisión quirúrgica completa, sin alteración o daño al nervio facial, con seguimiento postoperatorio de tres años sin recidiva.

Background In the pediatric population, dermoid cysts are among the most frequent lesions of the scalp and skull. Imaging plays a key role in characterizing scalp and skull lesions in order to narrow the differential diagnoses. In general, dermoids are described as heterogeneous T1-/T2-hypo- to hyperintense lesions on magnetic resonance imaging. Methods The goal of this retrospective study is to evaluate the diffusion weighted imaging findings while reviewing the conventional T1-/T2-/T1+C-weighted MR characteristics in a pathology-proven series of 14 dermoids of the pediatric scalp and skull. Results In our pediatric cohort (eight boys, six girls, age range 3–95 months), half of the dermoids were homogeneous T1-hypointense and homogeneous T2-hyperintense. We found a mixture of restricted (45.5%) and increased diffusion (54.5%) in dermoids. The vast majority of dermoids (91.7%) showed rim enhancement. Most dermoids (57.1%) were located at the midline and adjacent to one of its sutures. Conclusions This study suggests that dermoids may have more variable imaging appearances than hitherto assumed and are frequently seen in close proximity or adjacent to the anterior fontanelle.
