Pedunculated hepatocellular carcinoma and splenicmetastasis

Abstract: Only a few cases of pedunculated hepatocellular carcinoma (P-HCC) have been reported in the literature. The common sites of extrahepatic metastases in patients with HCC are the lungs, regional lymph nodes, kidney, bone marrow and adrenals. Metastasis to spleen is mostly via hematogenous metastasis, direct metastasis to spleen was very rare. We report a case of P-HCC presenting as a left upper abdominal lesions which involved the spleen that was actually a P-HCC with splenic metastasis. This case is unique as P… Show more

“…Therefore, the resection rate of P-HCC is high, but the prognosis is poor due to recurrence and metastasis to the peritoneum, adrenal glands, and spleen. 3 , 4 , 5 In our case also, it was difficult to diagnose P-HCC because it was mistaken for a pelvic mass. We describe a case of primary HCC that was detected incidentally during an obstetric operation.…”

“…[ 8 ] In addition, other factors may lead to splenomegaly in patients, such as splenic metastasis of carcinoma, splenic aneurysm, splenic tuberculosis, or splenic hemangioma. [ 9 – 12 ] For patients with tumors, whether there are other mechanisms that cause splenomegaly requires further study.…”

“…of HCC commonly occurs in lung (55%), regional lymph nodes (53%), kidney (42%), bone marrow (38%) and adrenal gland (11%) [2]. Metastasis to spleen is very rare and occurs due to direct extension, from free hematogenous spread or lymphatic invasion [3]. The incidence of splenic metastasis ranged from 0.7-0.8% disseminated cases [4].…”
