Peer-led HIV prevention among gay men in London: Process evaluation

Abstract: A peer-led HIV prevention initiative, based on a diffusion of innovation model, was developed for gay men attending gyms in central London. Peer educators were recruited from people who used the gym regularly according to standard selection criteria. After initial training, peer educators agreed to talk to gay men at their gym about HIV prevention, focusing on sexual risk and steroid injecting behaviour. Outcome evaluation revealed that the peer education programme had no significant impact on the risk behavio… Show more

“…The rationale was inspired by the diffusion of innovations (DOI) theory (Rogers, 2003) and the successful experience of respondentdriven sampling targeting MSM (Carballo-DieÂŽguez et al, 2011). The DOI theory has been widely used for developing HIV prevention targeting MSM in the USA and the UK (Bertrand, 2004;Elford, Sherr, Bolding, Serle, & Maguire, 2002;Kelly et al, 1991). As a test-of-concept study (Skare & Sietsema, 2005), we would like to test the feasibility and preliminary efficacy of this relatively new approach.…”

“…MSM gave a sense that they would have tested had they been further encouraged, an important missed opportunity. Missed opportunities in HIV testing, in both primary and secondary care, are widely recognised (Stekler & Golden, 2009 (Kelly et al, 1997;Elford et al, 2002;Flowers et al, 2002;).…”

“…Thus we would argue for process evaluation to be integrated into controlled trials from the outset to help us better understand how the outcome was achieved-or not. Our own research in the United Kingdom, evaluating the effectiveness of peer-led behavioral interventions, integrated outcome and process evaluations into the study design from the start (Elford, Sherr, Bolding, Serle, & Maguire, 2002;Flowers, Frankis, & Hart, 2000). This turned out to be vitally important because outcome evaluation revealed no intervention effects on the sexual risk behaviors of MSM (Elford et al, 2001b;Flowers et al, 2002).…”
