Penetrating injuries to the vertebral artery: interventions and outcomes from US Trauma Centers

Abstract: Penetrating injuries to the vertebral artery are rare and incompletely studied. Operative, angioembolic, and nonoperative strategies are management options, although the association between management strategy and outcomes is unknown. This study endeavored to define the epidemiology, management strategy, and outcomes after penetrating injuries to the vertebral artery presenting to trauma centers nationwide. Methods Patients with veterbral artery injuries were identified from the National Trauma Data Bank (NTDB… Show more

“…Traumatic vertebral artery injury (VAI), from hematoma with luminal stenosis to pseudoaneurysm (PSA), arteriovenous fistula (AVF), or complete occlusion, is reported in < 1% of blunt and penetrating trauma admissions [1][2][3][4] but is highly morbid, with a stroke risk of 6% to 38% 1 . The estimated mortality directly associated with traumatic VAI is 5-7% 1,3 , and when untreated can reach 70% for severe injuries 5 .…”

“…Los vasos más frecuentemente afectados son la vena yugular y la arteria carótida y sus ramas 1 . Por su parte, el compromiso de las arterias vertebrales es extremadamente infrecuente y notoriamente difícil de diagnosticar 2,3 sin los estudios diagnósticos adecuados. Esto debido a su localización profunda en el cuello y a su situación protegida por estructuras musculares y óseas en gran parte de su trayecto.…”
