Pentachlorophenol Intoxication: Report of a Fatal Case, with Comments on the Clinical Course and Pathologic Anatomy

Abstract: A case of a 33-yr-old man who died following occupational exposure to pentachlorophenol is presented. Postmortem examination revealed cerebral edema and fatty degeneration of the viscera. Review of the literature indicates that the clinical syndrome of poisoning with the compound results from mitochondrial toxicity with derangement of aerobic metabolism.

“…In 1967, a cluster of cases of critical illness in a newborn nursery occurred as a result of the misuse of sodium pentachlorophenate as an anti-mildew agent in the hospital laundry; nine poisoning cases and two fatalities were reported [25]. This and other accounts of accidental occupational exposures provide ample toxicologic evidence of the risks from acute exposure to PCP [26,27].…”

“…This effect is most pronounced in organ systems dependent on highperfusion and oxygen delivery, including the kidneys, heart, and CNS. On pathologic evaluation vacuolization of hepatocytes, proximal tubular cells, and myocardial tissue, as well as signs of mitochondrial injury have been shown in fatal cases involving neonates and adults [26,30]. Pentachlorophenol is also an inducer of cytochrome CYP3A [31], which could influence the metabolism of other drugs used in the critical care…”

“…The commercial or technical grade of PCP is more toxic to aquatic organisms than its pure or analytical grade. The commercial or technical grade of PCP is heavily contaminated with compounds including chlorophenols, dioxins, dibenzofiirans, hexachlorobenzene and phenoxyphenols (5). PCP and its sodium salt, sodium pentachlorophenate are primarily used as wood preservatives and secondarily as a herbicides, insecticides or bactéricides.…”
