Percutaneous aspiration versus tube drainage for spontaneous pneumothorax: systematic review and meta-analysis

Abstract: Spontaneous pneumothorax is an extremely frequent pathology. Despite this, there is still no clear consensus on managing these patients. Therefore, we perform a systematic review and meta-analysis of the effectiveness of percutaneous aspiration compared with tube drainage for treating idiopathic spontaneous pneumothorax. Controlled, randomised clinical trials were selected, in which the effectiveness of percutaneous aspiration is compared with that of chest tube drainage, in terms of resolution of the pneumoth… Show more

“…However, two meta-analyses (8,9) concluded that simple aspiration treatment showed rates of success equivalent to the chest tube drainage for the initial treatment of PSP. Ayed et al (10) published a randomized study comparing simple aspiration with tube thoracostomy in 137 patients with a first episode of PSP; 1-week success rates were 89% and 88%, and 1-year recurrence rates were 22% and 24%, respectively.…”

“…Chest tube drainage referred to the placements of one or more intercostal drainage tubes connected to water seal system or suction system, which yielded maintenance until air leak disappeared and the chest X-ray showed complete lung expansion (48). Generally speaking, chest tube drainages were usually applied for patients suffering from pneumothorax who should be admitted to hospital following the failure of needle aspiration (3).…”

“…are established procedures in the treatment of a PSP. Both procedures have been examined in several prospective randomized studies [34,104,[106][107][108], systematic review articles [109,111,113], and meta-analyses [110,113]. In a current Cochrane Review [117] of 6 RCTs, the primary treatment method (needle aspiration or small-lumen chest drain) showed a better primary success rate of drain application, while the hospitalization time following aspiration is shorter.…”
