Percutaneous CT-Guided Treatment of Lumbar Facet Joint Synovial Cysts

Abstract: Symptomatic intraspinal lumbar facet joint synovial cysts can be managed both conservatively and surgically. Diagnosis of the lumbar facet joint cyst is made through cross-sectional imaging of the spine, either by computerized tomography (CT) scan, myelography, or most commonly magnetic resonance imaging. Conservative treatment by facet joint injection can be performed under fluoroscopic or CT guidance, although only CT guidance provides direct visualization of the cyst confirming accurate needle placement. “…Intraspinal synovial cysts are continuous with the facet joints and lined with epithelial lining. They are typically located posterior to the thecal sac; this is thought to be related to the presence of ligamentum flavum which represents a barrier to anterior cyst formation 3. Majority of these cysts occur in the segments of lumbar spine with the predilection for the fourth and fifth lumbar vertebra as described in our patient 1,46.…”

“…Consequently formation of the cavity filled with synovial fluid occurs 1,2,5. Therefore, the pathognomic imaging finding is a direct communication between the cyst and the facet joint 3,5. This was clearly demonstrated in our case as well.…”

“…6 Facet joint synovial cyst rupture using single-needle distention has been reported in the literature; however, attempts were not successful in all cases. [6][7][8][9][10][11][12][13][14] In a retrospective review involving 101 patients with symptomatic facet cysts who underwent single-needle percutaneous rupture, Martha et al 6 reported a failure rate of approximately 20%.…”
