Perforation of the Interventricular Septum in Myocardial Infarction

Abstract: Rupture of the interventricular septum occurring as a complication of myocardial infarction has been reported with increasing frequency. The condition, at one time thought to be rare, is not uncommon and is of prognostic importance. It is, however, less common than rupture of the ventricular wall, for in a series of 698 cases ofcoronary occlusion, 6 per cent suffered from this latter complication (Bean, 1938). Prior to 1940 there were isolated reports of single cases, though Sager (1934) collected eighteen tha… Show more

“…I n recent years further cases including several diagnosed ante mortem have been reported from Scandinavia (4,12,18,19,20,21,26,30,32) and other countries. Series of ruptures of the septum many of which were diagnosed before death have also been published (2,13,17,24,25,31). All these series, however, probably represent a certain selection.…”

“…GLN presented with the most common form of post infarction VSD occurring in up to 60% of cases of post infarction VSD (6). She also typically presented with the risk factors for VSD formation; the female sex, totally occluded single infarct related vessel and no previous history of smoking or heart disease (7-9), she was however a diabetic.…”

“…The onset of a new systolic murmur after myocardial infarction is the clinical sign that caused clinicians to suspect ventricular septal ruptue. In rare instances, a systolic murmur is absent or overlooked 15 ) . The diagnosis of ventricular septal rupture by means of bedside right-heart catheterization and sequential oximetry was described in 1972 16 ) .…”
