Background Polysomnography (PSG) is the gold standard for the diagnosis of obstructive sleep apnoea (OSA). Home sleep apnoea testing with peripheral arterial tonometry (PAT) is a recommended diagnostic alternative for patients with an increased risk for OSA. In a large clinical cohort, we investigated concordance and predictors for discordance in diagnosing OSA using PAT and PSG, and three-year cardiovascular risk in patients with discordant OSA diagnosis. Methods Retrospective monocentric cohort study. Patients with a PAT AHI ≥ 5/h followed by an in-hospital PSG within three months were included. All patients with a PAT AHI ≥ 5/h but a PSG AHI < 5/h were classified as discordant. Patients with PAT and PSG AHI ≥ 5/h were classified as concordant. To ascertain cardiovascular risk, major adverse cardiovascular events (MACE) were analyzed in discordant patients and sex, age, body mass index (BMI) and cardiovascular disease-matched concordant patients over a follow-up time of 3.1 ± 0.06 years. Results A total of 940 patients, 66% male with an average age of 55 ± 0.4 years and BMI of 31 ± 0.2 kg/m2 were included. Agreement in OSA diagnosis was observed in 80% of patients (55% in mild and 86% in moderate and severe OSA). Factors significantly associated with a discordant diagnosis were female sex, younger age and lower BMI, but not comorbidities. There was no significant difference in MACE (p = 0.920) between discordant patients (n = 155) and matched concordant patients (n = 274) with or without therapy. Conclusions Concordance between PAT and PSG diagnosis of sleep apnoea is good, particularly in moderate and severe OSA. Predictors for discordant results between PAT and PSG were age, sex and BMI. MACE risk is similar in those with OSA diagnosed by PAT or PSG.

Background: In addition to the assisted ventilation method (PAP) for the therapy of OSA, the mandibular advancement device (MAD) has been available since 1984 by Meier-Ewert et al. as an alternative treatment method in the dental field. Since 01.01.2022, it can be prescribed at the expense of the statutory health insurance in Germany. It is not possible to ensure the quality and safety of the treatment procedures MAD (TPMAD) simply by following the medical device-independent recommendations and guidance and treatment guidelines. Due to different designs, a product-related assessment procedure with declaration of conformity (MDRC) is additionally required for the manufacturing and treatment process in accordance with MDR (EU Regulation 2017/745) on medical devices regarding quality (performance) and safety, as well as the maintenance of a quality management system oriented towards ISO 13485. To date, the literature lacks information on the methodology of TPMAD from the MDRC of a medical product (MP). The aim of this work is their first and exemplary description. Method:The following methodology was chosen for the TPMAD in the MDRC of the MP F-UPS ® : •The Vector Diagram "Risk Profile" (RP) as a diagnostic tool for dental risk assessment. •The 3D Bite Fork "JS-Gauge ® " (Unique Device Identification, UDI 4260731570017T) (JG) as the bite fork for the registration of the start protrusion position (SPP). •The MAD "F-UPS ® " (UDI 426073157F-UPS3C) (F) for the therapy of OSA. •The Vector Diagram "Performance Monitor" (PM) as an evaluation tool for assessing effectivenessResults: This paper is the first to provide a contribution to the TPMAD methodology from the MDRC of an MP.The methodical individual steps with the RP, JG, F and PM map the complete TPMAD in a MDR compliant fashion. Through the standardization of the methodology, the assurance of quality and safety can be implemented effectively, efficiently and satisfactorily MDR compliant.

PurposeThis review aims to highlight the pivotal role of the mandibular jaw movement (MJM) signal in advancing artificial intelligence (AI)‐powered technologies for diagnosing obstructive sleep apnea (OSA).MethodsA scoping review was conducted to evaluate various aspects of the MJM signal and their contribution to improving signal proficiency for users.ResultsThe comprehensive literature analysis is structured into four key sections, each addressing factors essential to signal proficiency. These factors include (1) the comprehensiveness of research, development, and application of MJM‐based technology; (2) the physiological significance of the MJM signal for various clinical tasks; (3) the technical transparency; and (4) the interpretability of the MJM signal. Comparisons with the photoplethysmography (PPG) signal are made where applicable.ConclusionsProficiency in biosignal interpretation is essential for the success of AI‐driven diagnostic tools and for maximizing the clinical benefits through enhanced physiological insight. Through rigorous research ensuring an enhanced understanding of the signal and its extensive validation, the MJM signal sets a new benchmark for the development of AI‐driven diagnostic solutions in OSA diagnosis.
