Performance profile of FDG-PET and PET/CT for cancer screening on the basis of a Japanese Nationwide Survey

Abstract: We were able to clarify the performance profile of "FDG-PET for cancer screening" on the basis of a Japanese nationwide survey. The number of facilities possessing PET is increasing steadily, highlighting the necessity of evaluating the usefulness of "FDG-PET cancer screening" as soon as possible by undertaking long-term investigations of large series of subjects.

“…This may be due to our close examination of all focal PET incidentalomas using US and FNAB and the long-term follow-up. The mean age of 1,501 subjects in our study was 43.7 years old, about 10 years younger than in the study by Chen et al (52.8 years old) and Minamimoto et al (the median age corresponded to the group of 50-59 years old) [6,7]. Given that the incidence of thyroid nodule and thyroid cancer increases with age [8], it is surprising that we found a higher incidence of thyroid cancer in our relatively younger subjects.…”

“…In our current study, thyroid cancer was proven in 11 of 20 focal PET incidentalomas in 1,501 healthy subjects. The cancer risk was 55%, markedly higher than in previous reports [1,6,7]. This may be due to our close examination of all focal PET incidentalomas using US and FNAB and the long-term follow-up.…”

“…Although PET has recently been proposed as a promising cancer screening test [47,48], the utility of PET in gastric cancer screening has not been evaluated. For gastric cancer screening, the usefulness of PET is limited because of physiological FDG uptake in the normal gastric wall and differences of FDG uptake according to the histological type of the tumor [1,2].…”

“…According to a multi-institutional study conducted by the Japanese Society of Nuclear Medicine and the Japan Radioisotope Association for cancer screening including PET, 30 cases of gastric cancer were found in 50,558 healthy subjects [47]. PET showed a relative sensitivity of 27% and PPV of 16%.…”
