Perinatal and Neonatal Outcomes of Triplet Gestations Based on Placental Chorionicity

Abstract: The purpose of our study was to evaluate perinatal and neonatal outcomes in triplet gestations in relation to placental chorionicity. We hypothesized that triplets containing a monochorionic pair (dichorionic triamniotic) would have increased morbidity compared with triplets without a monochorionic pair (trichorionic triamniotic). We retrospectively analyzed all triplet sets > or =20 weeks delivering at our institutions from January 1995 through April 2007. Data were collected via perinatal and neonatal databa… Show more

“…Comparison of gestational age at birth between TCTA and DCTA triplets was reported by five studies. Overall, there was no difference in gestational age at birth between TCTA and DCTA triplets (MD, 1.1 weeks (95% CI, –0.3 to 2.5 weeks); I 2 = 85%; P = 0.12).…”

“…Seven ( n = 2754) and four ( n = 783) studies reported the incidence of morbidity in TCTA and DCTA triplets, respectively. Neurological morbidity occurred in 2.0% (95% CI, 1.1–3.3%) of TCTA and in 11.6% (95% CI, 1.1–40.0%) of DCTA triplets (Table ).…”

“…Nevertheless, 51 of these pregnancies resulted in a dichorionic triplet gestation. It has been demonstrated that dichorionic triplets are at increased risk for earlier deliveries and have higher perinatal mortality rates compared with trichorionic triplets [1] [14]. It is therefore imperative to try to understand what factors contribute to embryo splitting in this population.…”

“…In this study, groups based on trio and on triplet zygosity and chorionicity were compared on gestational age, as it has been proposed that, with less chorions in a triplet pregnancy, gestational age is lower (Spencer et al, 2009 the trios of which more triplets had shared a chorion. Although no significant effects were found for lower gestational age, this characteristic was entered in the regression analysis, to correct for even small effects.…”
