Less than 6% of rare illnesses have an appropriate treatment option. Repurposed medications for new indications are a cost-effective and time-saving strategy that results in excellent success rates, which may significantly lower the risk associated with therapeutic development for rare illnesses. It is becoming a realistic alternative to repurposing "conventional" medications to treat joint and rare diseases considering the significant failure rates, high expenses, and sluggish stride of innovative medication advancement. This is due to delisted compounds, cheaper research fees, and faster development time frames. Repurposed drug competitors have been developed using strategic decisions based on data analysis, interpretation, and investigational approaches, but technical and regulatory restrictions must also be considered. Combining experimental and computational methodologies generates innovative new medicinal applications. It is a one-of-a-kind strategy for repurposing human-safe pharmaceuticals to treat uncommon and difficult-to-treat ailments. It is a very effective method for discovering and creating novel medications. Several pharmaceutical firms have developed novel therapies by repositioning old medications. Repurposing drugs is practical, cost-effective, and speedy and generally involves lower risks when compared to developing a new drug from the beginning.

Chronotherapy for hypertension has been a topic of debate over the past decade. A number of studies up to 2019 suggested that prescribing drugs in the evening has advantages compared to morning, both in terms of normalization of blood pressure and its circadian rhythm, as well as in terms of the risk of adverse outcomes. Some of these studies were not designed for such an analysis, and some have significant limitations that make their interpretation difficult. The results of the HARMONY and TIME studies published in the last 2 years did not confirm any advantage of taking drugs in the evening compared to the morning, but showed its safety. Selection of antihypertensive therapy should be guided by antihypertensive efficacy, duration and stability of the effect, as well as medical adherence. Therefore, a single intake of double or triple fixed-dose combination of long-acting agents at hours convenient for the patient is optimal.

Hypertension (HTN) is a major modifiable cardiovascular risk factor. Despite availability of effective blood pressure-lowering medications, HTN remains poorly controlled. Causes of suboptimal HTN control include the asymptomatic nature of HTN, low patient compliance, inadequate knowledge regarding the sequelae of HTN, and often complex therapeutic regimen. In the current guidelines, combination therapy using single pill combinations (SPC) has been widely recommended for the treatment of HTN. In the present paper, we evaluated the usefulness of perindopril-based SPC in various clinical scenarios in everyday practice. Perindopril-based SPC, including both two-and three-drug combinations, have been marketed since 2008. Our considerations regarding the use of perindopril-based combinations both within the basic HTN management strategy and for treatment individualization are informed by not only the everyday clinical experience but also the results of numerous clinical studies, including large randomized trials. Perindopril-based SPC are characterized by a long duration of action providing stable 24-hour blood pressure control, and the wide range of perindopril-based SPC allows their use in various clinical scenarios, thus facilitating treatment individualization.
