Perioperative assessment of platelet function by Thromboelastograph® Platelet Mapping™ in cardiovascular patients undergoing non-cardiac surgery

Abstract: According to the findings, the TEG PM assay might be a feasible approach to objectively evaluate the effects of aspirin and clopidogrel during the perioperative period and potentially guide drug management.

“…Other reports have stated the use of different parameters provided by these tests are predictors of both postoperative bleeding and blood product use [143][144][145] . Thrombelastograph Platelet Mapping System has been developed to monitor antiplatelet therapy [146][147][148][149] . A weak clot is formed by the addition of reptilase and factor XIII, by adding AA or ADP the clot strength is increased allowing this assay to be sensitive to dual antiplatelet therapy.…”

“…Platelet aggregation is initiated through adding arachidonic acid (AA) or adenosine diphosphate (ADP) to a sample of blood and the effects of platelet activation on blood clot formation are analysed. Identification of antiplatelet medication is enabled through the use of TEG-PM [40]. For assessment of platelet function through the POC device Platelet Function Analyser (PFA)-100 (Siemens Diagnostics, Deerfield, IL, USA) a sample of citrated whole blood is aspirated at shear rates through disposable cartridges.…”

“…Thromboelastography (TEG) is widely used as a monitoring technique for hemostasis in surgical procedures 10 11. It is performed in whole blood and assesses clot formation and can be used to test the inhibitory response to antiplatelet agents such as aspirin and clopidogrel 12–14.…”
