Perioperative Care for Kidney Transplant Recipients

Abstract: Transplantation carries significant mortality benefit compared to dialysis in end-stage kidney disease. Increased perioperative risk, however, results in a higher mortality in the first 3 months post-transplantation compared to remaining on haemodialysis. Consequently, optimal perioperative management is essential. Patients presenting for kidney transplantation require rapid assessment and preparation for theatre to minimise ischaemic times and improve mortality and graft outcomes. This task is often complicat… Show more

“…The stress related to surgery and the use of high doses of steroids during the induction period frequently induce hyperglycemia in the early post-transplant period. Insulin infusion is used perioperatively, as appropriate, to control hyperglycemia [55, 56]. In one retrospective study, the development of perioperative hyperglycemia after transplantation significantly predicted a 4-fold increase in the risk of PTDM at 1-year post-transplant [57].…”

“…This is reflected in Canadian statistics showing a 41% increase in the total number of kidney transplants performed from 2010 and 2019. 1 Despite increased perioperative risk and higher mortality in the first 3 months posttransplantation compared with remaining on hemodialysis, 2 patient survival and quality of life after transplant are improved in the long term, as are overall costs. 3,4 Posttransplant complications affect long-term graft and patient outcomes.…”
