Perioperative covert stroke in patients undergoing coronary artery bypass graft surgery

Abstract: Objectives: Covert stroke is a complication of coronary artery bypass graft surgery that is increasingly recognized as a serious problem. In noncardiac surgery settings, covert stroke is associated with the development of delirium, long-term cognitive decline, and future clinical stroke. Therefore, we sought to determine the feasibility of conducting a large, prospective cohort study of the influence of covert stroke on neurocognitive outcomes in patients undergoing coronary artery bypass graft surgery. Methods… Show more

“…Coronary artery bypass grafting (CABG) is a common factor for a post-operative delirium [ 81 ]. Moreover, post-operative delirium after a CABG is more frequent in patients with compared with those without covert stroke [ 82 ]. Consequently, CAD may constitute an additive risk for delirium in stroke patients compared with non-stroke patients.…”

“…The analysis was performed as a feasibility study for future large multicenter prospective studies of covert stroke after CABG. 3 Such a study would be both valuable and, as the current study suggests, feasible.…”

“…In the current article Browne and colleagues 3 analyze the association of covert stroke (asymptomatic acute infarcts on magnetic resonance imaging [MRI]) after CABG with intraoperative cerebral oxygen desaturation events and postoperative cognitive function. Of the 49 of 66 patients who completed an MRI and had interpretable images, 39% had a covert stroke and 6% a clinical stroke.…”

“…In this issue of the Journal , Browne and colleagues 1 report the incidence and describe the importance of covert stroke during coronary artery bypass surgery (CABG) in 49 patients who had diffusion-weighed magnetic resonance imaging (DW-MRI) of the brain, showing 39% with perioperative covert stroke, 6% with clinical stroke, 26% with delirium, and 10% with no stroke. These are important findings for such a routine procedure.…”
