CT is increasingly being used for evaluating the cardiovascular structures and airways in the patients with congenital heart disease. Multi-slice CT has traditionally been used for the evaluation of the extracardiac vascular and airway abnormalities because of its inherent high spatial resolution and excellent air-tissue contrast. Recent developments in CT technology primarily by reducing the cardiac motion and the radiation dose usage in congenital heart disease evaluation have helped expand the indications for CT usage. Tracheobronchomalacia associated with congenital heart disease can be evaluated with cine CT. Intravenous contrast injection should be tailored to unequivocally demonstrate cardiovascular abnormalities. Knowledge of the state-of-the-art CT imaging techniques that are used for evaluating congenital heart disease is helpful not only for planning and performing CT examinations, but also for interpreting and presenting the CT image findings that consequently guide the proper medical and surgical management.

Retroesophageal aortic arch, in which the aortic arch crosses the midline behind the esophagus to the contralateral side, is a rare form of vascular anomaly. The complete form may cause symptoms by compressing the esophagus or the trachea and need a surgical intervention. We report a rare case of a hypoplastic left heart syndrome variant with the left retroesophageal circumflex aortic arch in which the left aortic arch, retroesophageal circumflex aorta, and the right descending aorta with the aberrant right subclavian artery encircle the esophagus completely, thus causing central bronchial compression. Bilateral pulmonary artery banding and subsequent modified Norwood procedure with extensive mobilization and creation of the neo-aorta were performed. As a result of the successful translocation of the aorta, the airway compression was relieved. The patient underwent the second-stage operation and is doing well currently.

Due to recent advances in multidetector computed tomography (MD-CT) technology, MD-CT images allow evaluation of both extracardiac and intracardiac anatomy. The images provide high-quality spatial and temporal resolution and are highly reproducible irrespective of acoustic window parameters. MD-CT has been shown to be very useful in the field of congenital cardiovascular malformations, especially in complex anomalies. Furthermore, it can less invasively provide information about not only target lesions but also their surrounding structures including airways and their spatial relationships.Nowadays, MD-CT provides surgeons and catheter interventionalists with precise diagnostic information as well as important information for both preoperative and postoperative management and catheter intervention.The clinical role and usefulness of MD-CT in the evaluation of children and adults with congenital heart disease for both preoperative and postoperative management and catheter intervention are discussed and illustrated with several examples.
