Peripapillary and macular retinoschisis in a patient with pseudoexfoliation glaucoma

Abstract: A 55-year-old man was admitted with a 2-year history of pseudoexfoliation glaucoma. The best-corrected visual acuities were 10/10 in both eyes. Pseudoexfoliation material was noted on the iris and the lens capsule. Fundus examination evidenced cup-to-disc ratio of 0.7 OD and 0.9 OS with peripapillary atrophy. Gonioscopy revealed open angles. Optical coherence tomography demonstrated peripapillary retinoschisis extending to the nasal macula in the left eye and optic nerve head was almost fully excavated. Coexis… Show more

“…Peripapillary and macular schisis has been described in POAG[ 13 ], sometimes associated with serous detachment in eyes with a large optic nerve head cup[ 14 ], pseudoexfoliation glaucoma [ 15 ] and normal tension glaucoma. [ 8 ] The differential diagnosis of splitting in our patients includes myopic foveoschisis, tractional retinoschsis and traction on the disc.…”

“…Peripapillary retinoschisis is characterized by the splitting of the peripapillary retinal nerve fiber layer. Previously, several case reports have described peripapillary retinoschisis in patients with different types of glaucoma, including primary open angle glaucoma, normal-tension glaucoma, narrow angle glaucoma, angle-closure glaucoma, and pseudoexfoliation glaucoma [ 8 12 ]. However, the clinical features were variable and the pathogenic mechanisms were different.…”

“…Although nasal and peripapillary retinoschisis have been demonstrated in patients with glaucoma, involvement of nasal retina is not well known in XLRS. [ 4 5 ]…”
