Peripheral atherectomy in TransAtlantic InterSociety Consensus type C femoropopliteal lesions for limb salvage

Abstract: Peripheral atherectomy can achieve good early clinical and hemodynamic success in patients with TASC C lesions and critical limb ischemia. However, mid-term restenosis rates are high in this challenging cohort of patients.

“…7,8,[15][16][17] These discrepancies likely can be attributed to patient selection, severity of disease, and method of follow-up. For example, similar to our 16 reported higher than expected amputation rates (18% and 30%) and lower patency rates, while Keeling et al 15 reported 1-year primary patency rates of 62% and an amputation rate of 7%.…”

“…There have been numerous reports documenting promising early results of this device to treat lower extremity occlusive disease; 6,7 however, other studies have questioned its utility. 8,9 In an effort to determine which patients will benefit from atherectomy, we reviewed our experience and report midterm results and predictors of outcomes in infra-inguinal lesions treated with the SilverHawk atherectomy catheter.…”

“…The Silverhawk device is currently used in endovascular interventions for the treatment of symptomatic PAD and appears to be a viable tool [76][77][78][79][80][81][82]. For the CLI population specifically, the Silverhawk device can be safely used either as an adjunctive tool or as stand-alone therapy [83].…”

“…For the CLI population specifically, the Silverhawk device can be safely used either as an adjunctive tool or as stand-alone therapy [83]. This was supported by a small series evaluating TASC-type C femoropopliteal lesions in CLI showing that atherectomy achieved acceptable early results [78]. A potential complication of directional atherectomy is embolization although the TALON registry reported a very low rate of embolization of 0.1% [81].…”

“…[1][2][3] The initial success rate of EVT is high, and using new devices is associated with better outcomes after EVT. [4][5][6] However, restenosis is often seen during the follow-up phase after EVT due to a combination of elastic recoil, negative remodeling, or hyperplasia. 7,8 Thus, restenosis is a clinical limitation after EVT.…”
