Peritoneal mesothelioma: Sonographic findings in nine cases

Abstract: Ultrasonographic findings in nine cases of peritoneal mesothelioma are presented. The most common findings were sheet-like or nodular peritoneal thickening, soft tissue masses, fixation of the intestinal loops, mesenteric thickening, and minimal ascites which was disproportional to the degree of tumor dissemination. The authors found that abdominal sonography, using 3.75-7.5 MHz transducers, is a useful imaging method for diagnosis of peritoneal mesothelioma in high-risk groups.

“…Ascites, peritoneal thickening, omental infil tration, and adhesions are also features of primary or secondary tumors of the peritoneum. In mesotheli oma, the ascites is disproportionately minimal in re lation to the degree of tumor dissemination (3). The adhesive bands between the parietal peritoneum and intestinal loops are often thick and rigid in perito neal malignancies, unlike the fine, mobile, complete and incomplete septations in TBP (1,3).…”

The adhesive bands between the parietal peritoneum and intestinal loops are often thick and rigid in perito neal malignancies, unlike the fine, mobile, complete and incomplete septations in TBP (1,3). Although delicate septations may also be present in pyogenic peritonitis and hemoperitoneum, the US appearance of the ascites fluid changes rapidly with time, unlike the ascites of peritoneal tuberculosis.…”

“…Differential diagnosis mainly includes peritoneal carcinomatosis, primary peritoneal mesothelioma, peritonitis, and rarely lymphoma [27,29,36,37,41]. Differential diagnosis between peritoneal tuberculosis and peritoneal carcinomatosis is still challenging and has a crucial importance, as peritoneal tuberculosis, if promptly diagnosed, has an effective therapy and good survival rates [39].…”

“…The suggestive features in the patients with peritoneal mesothelioma consist of multifocal peritoneal thickening of either sheet-like or nodular types up to 3 cm, omental or mesenteric or peritoneal soft tissue masses, thick-rigid septa between peritoneal leaves and fixed bowel loops, and disproportionately small amount of ascites to the degree of tumor dissemination. Whereas suggestive features for the diagnosis of peritoneal tuberculosis include the presence of a smooth, minimal peritoneal thickening (< 5 mm) with pronounced enhancement, and multiple, fine, complete, or incomplete, mobile septa [29,30,40,41].…”
