Persistent interferon‐β‐1b‐induced psychosis in a patient with multiple sclerosis

Abstract: Interferon-ß is used in patients with multiple sclerosis to reduce autoimmunity; although other psychiatric side-effects are common, in contrast to interferon-alpha, psychosis has been reported only once. A patient with multiple sclerosis developed auditory hallucinations, paranoid delusions, and increased aggressiveness after 16 months of treatment with interferon-ß-1b, 250 mg every other day. “…The initial presentation of BD may include a depressive phase, or a manic-phase with or without psychotic symptoms. All of these presentations have been temporally associated with MS (13–17) and pharmacological treatments of MS (18, 19), making expeditious and confident diagnosis perilous. It remains unclear to what degree and how consistent bipolar disorder is as a prodromal feature of MS.…”

“…There is also a case report of persistent psychosis due to interferon-β-1b in a patient with MS (Manfredi et al, 2010). However, to the best of our knowledge there are no reported cases with natalizumab, and it is unlikely that this medication caused the auditory phenomenon because cessation for a few months did not abate the symptoms.…”
