Persistent involvement of anterior mesorectal fascia in carcinoma rectum – extended resection of rectum vs total pelvic exenteration: results from a single‐centre retrospective study

Abstract: Although there is no significant difference in the overall survival and disease-free survival, ERR is associated with a high risk of local recurrence compared to TPE and the downstaged group.

“…This contradicted the findings of the MERCURY II trial, in which tumours located anteriorly independently affected the pathological CRM status 26 . In another retrospective study, 23.1% of patients with persistent involved anterior MRF after neoadjuvant radiation, developed local recurrence despite an R0 resection 27 . Interestingly, the local recurrence rate in the present study was 7.3%, which is lower than for peritoneal and distant failures, following a similar pattern with those undergoing exenterative surgeries 21 .…”

“…4 Besides, the existing literature also demonstrated the limited survival benefit after TPE. 6 With the development of pre-operative CRT for down-staging, bladder-sparing surgery, which can achieve acceptable oncological and functional outcomes, has become increasingly popular in recent years. 5 When the seminal vesicle is invaded, unilateral or bilateral en-bloc seminal vesicle resection (SVR) could be performed.…”

“…However, for lesser involvement of the SV or involvement of the anterior mesorectal fascia at the level of SV, a total pelvic exenteration can be avoided without threatening the circumferential margins. 2 In the video presented, we describe our technique of isolated en-bloc SV excision with total mesorectal excision (TME) in a case of locally advanced rectal cancer. We highlight the technical consideration required to achieve oncological safety and avoid damage to urologic organs.…”
