Persistent omental trophoblastic implantation following salpingostomy, salpingectomy and methotrexate for ectopic pregnancy: A case report

Abstract: 2% of all pregnancies are ectopic. Optimal surgical management is currently salpingectomy over salpingostomy, secondary to the risks of persistent trophoblastic tissue or omental implants (15%). However, rare cases of omental trophoblastic implants following laparoscopic salpingectomy have been noted in the literature. Current practice dictates that serial determinations of human chorionic gonadotropin (beta-hCG) levels after salpingectomy are not required, as it is considered a definitive treatment. However, … Show more

“…However, it is advised to do serial B-HCG measurements after methotrexate or salpingotomy to confirm completion of treatment 5 7. Most cases that have been reported so far in the literature, were related to salpingotomy rather than salpingectomy 8. Yet, we managed to present a summary of literature review for all cases with persistant ectopic pregnancy after salpingectomy(table 1).…”

“…In another case, salpingotomy was attempted but ended with salpingectomy in the same procedure, the patient presented later with pain and high BHCG and received methotrexate treatment. However, she developed intra-abdominal bleeding after that and had a laparoscopy to control the bleeding and remove the implants 8…”

“…The need for a second laparoscopy should be based on symptoms rather than changes in concentrations of human chorionic gonadotrophin. In a randomised controlled trial, methotrexate together with laparoscopic salpingostomy was found effective in reducing the complication of persistent trophoblast [43] .…”
