Persisting myelin oligodendrocyte glycoprotein antibodies in aquaporin-4 antibody negative pediatric neuromyelitis optica

Abstract: Pediatric patients presenting with clinical symptoms and MRI findings highly suggestive of NMO but with high and persisting MOG-IgG antibody titers are most likely to represent a distinct subgroup of acute demyelinating diseases with important clinical and therapeutic implications.

“…Anti-MOG antibodies occur more frequently in children than in adults and have been described (using different assays which, as noted above, may affect findings) in pediatric patients with a variety of autoimmune demyelinating disease phenotypes, including those of ADEM, MS, 32,[35][36][37] relapsing ADEM, relapsing ON, 38,39 relapsing ADEM-ON, 40 and relapsing ON-longitudinally extensive TM (NMO-like). 36,37 The normal function of MOG, which resides at the cell surface of oligodendrocytes, is not well understood.…”

“…[14][15][16][17][18] In contrast, the clinical relevance of MOG antibodies in adults is unclear, as only a minority of patients with MS and NMO/NMOSD are seropositive. 16,[18][19][20][21][22][23][24] Herein, we provide evidence that MOG antibodies are a clinical biomarker of bilateral and/ or recurrent optic neuritis (BON) in adults and describe the characteristic clinical course, response to therapy, and visual outcomes of this condition.…”

“…6 It was shown that some children with ADEM had high titers of MOG antibodies (≥1:160) that rapidly decreased to the undetectable levels after the first attack. 7,8 Strikingly, persistence with fluctuations of anti-MOG IgG was noted in children with MS. 7 MOG antibodies rapidly declined in patients who completely recovered from ADEM. Decreasing MOG antibody titers show favorable clinical outcome in patients with ADEM.…”

“…Rostasy et al 8 showed that the presence of high-titer anti-MOG IgG in a subgroup of patients with recurrent ON as compared with children with monophasic ON. In their study, the patients with recurrent attacks needed to be treated with immunomodulators, including AZA, rituximab, and IVIG.…”
