Phacoemulsification with intraocular pinhole implantation associated with Descemet membrane endothelial keratoplasty to treat failed full-thickness graft with dense cataract

Abstract: We present a case in which a different approach was used to treat an eye with dense cataract associated with an irregular failed full-thickness corneal graft. After cataract surgery, a foldable intraocular pinhole was implanted in the capsular bag to treat the irregular corneal astigmatism. Next, a Descemet membrane endothelial keratoplasty graft was used to improve transparency in the failed penetrating keratoplasty. This approach addressed the cataract, irregular cornea, and failed graft through a standard 2… Show more

“…With 4.25 D of corneal cylinder and an UNVA of 6/19 postoperatively, the patient was 'delighted' with the functional range of vision provided in their work as a truck driver. The advantage of the pin hole design in the treatment of irregular corneal astigmatism is also well documented with the XtraFocus pin hole IOL (Morcher, Stuttgart, Germany) [18][19][20][21]. The XtraFocus implant is a black, hydrophobic acrylic sulcus IOL that can be used in pseudophakic eyes, masks a greater area which may help with pupil abnormalities such as Aniridia and is translucent to infrared imaging.…”

“…The pinhole effect has been already exploited by the add-on XtraFocus lens (Morcher), a total black IOL for sulcus implantation, specifically designed for cases with corneal irregularities. [11][12][13] Compared to the XtraFocus lens, which is an add-on lens without optical power, the IC-8 has optical power and can be placed directly in the capsular bag during the cataract surgery, with a 1.36 mm central aperture and a non-diffractive 3.23 mm diameter opaque mask that allows the possibility of studying the retina, and also performing retinal surgery.…”

“…Small-aperture IOLs have been designed to address cases with irregular corneal aberrations and have been documented to enhance the functional vision and impart extended depth of focus. [1][2][3] These IOLs can be placed intraocularly; however, their availability is often an issue in many countries. Under such circumstances, PPP can be performed, and the benefits of pinhole optics can be availed.…”

“…Principles of small-aperture optics can be applied to cases with HOAs, which are refractory to other forms of currently available treatment. [2][3][4][5] The placement of an IC-8 IOL (Acufocus, Inc.) in post-RK case has been demonstrated to help achieve spectacle independence at distance, intermediate, and near. 1 In this study, we describe a safe and an effective technique of performing pinhole pupilloplasty (PPP) with single-pass 4-throw (SFT) method in the setting of previous RK using Purkinje 1 (P1) image as the marker for centration.…”
