Pharmacological Studies with 5-Fluorocytosine

Abstract: A cylinder plate bioassay for 5-fluorocytosine (5-FC) is described which permits determination of 5-FC concentrations in biological fluids in the presence of amphotericin B. Using this assay, we determined serum concentrations in 12 patients after a single dose of drug and in 8 patients receiving daily 5-FC at 6-hr intervals (4 to 120 days). Drug was detectable in serum as early as 0.5 hr and concentrations were measurable as long as 24 hr after a dose, regardless of renal function. Peak concentrations occurre… Show more

“…This level was achieved for periods up to 3 h after a single dose of 5-FC in this study. The serum concentrations of 5-FC achieved in dogs were roughly equivalent to those expected in humans given similar milligram-per-kilogram doses (3). Therefore, the excellent penetration of 5-FC into bronchial secretions indicates potential clinical efficacy for 5-FC in intrabronchial candida therapy.…”

“…The concentrations of 5FC in human CSF are similar to its corresponding serum concentrations (91,96,250), and a combination of 5FC with AmBd or L-AMB is a recognized first-line induction therapy for cryptococcal meningitis (97).…”

“…5-Flucytosine does not bind to plasma proteins and possesses excellent penetration into the CSF: 1 to 2 h after dosing, CSF concentrations amounted to 74.4% Ϯ 5.6% of the corresponding serum levels (23).…”
