Photodynamic therapy (PDT) is a medical treatment used to target solid tumors, where the administration of a photosensitizing agent and light generate reactive oxygen species (ROS), thus resulting in strong oxidative stress that selectively damages the illuminated tissues. Several preclinical studies have demonstrated that PDT can prime the immune system to recognize and attack cancer cells throughout the body. However, there is still limited evidence of PDT-mediated anti-tumor immunity in clinical settings. In the last decade, several clinical trials on PDT for cancer treatment have been initiated, indicating that significant efforts are being made to improve current PDT protocols. However, most of these studies disregarded the immunological dimension of PDT. The immunomodulatory properties of PDT can be combined with standard therapy and/or emerging immunotherapies, such as immune checkpoint blockers (ICBs), to achieve better disease control. Combining PDT with immunotherapy has shown synergistic effects in some preclinical models. However, the value of this combination in patients is still unknown, as the first clinical trials evaluating the combination of PDT with ICBs are just being initiated. Overall, this Trial Watch provides a summary of recent clinical information on the immunomodulatory properties of PDT and ongoing clinical trials using PDT to treat cancer patients. It also discusses the future perspectives of PDT for oncological indications.

Cancer has been one of the most serious diseases, resulting in more than 10 million deaths every year. Fiber-optic sensors have great potential for diagnosing and treating cancer due to their flexibility, precise positioning, real-time monitoring, and minimally invasive characteristics. Compared to traditional central laboratory examination, fiber-optic biosensors can provide high sensitivity, miniaturization, and versatility, which feature the point-of-care diagnostic capability. Herein, we focus on recent advances in fiber-optic biosensors for cancer theranostics. It is primarily concerned with advancements in the design of various fiber sensing approaches, fiber cancer sensing, and therapy sensors. With fiber-optic biosensors, cancer marker detection, cancerous cell differentiation, ex vivo tumor model validation, and in vivo tumor detection can be achieved. And the medical fiber also can be used to provide photothermal therapy, photodynamic therapy, and combination therapy for solid tumors. Additionally, cancer sensing and therapy can be integrated into the fiber, which demonstrates the multiplexing capabilities of fiber-optic biosensors. Lastly, we systematically summarize the fiber biosensor applications from in vitro to in vivo, and conclude with the challenges in development and prospects.

Photodynamic therapy (PDT) has been used clinically to treat cancer for more than 40 years. Some solid tumors, including esophageal cancer, lung cancer, head and neck cancer, cholangiocarcinoma, and bladder cancer, have been approved for and managed with PDT in many countries globally. Notably, PDT for gastric cancer (GC) has been reported less and is not currently included in the clinical diagnosis and treatment guidelines. However, PDT is a potential new therapeutic modality used for the management of GC, and its outcomes and realization are more and more encouraging. PDT has a pernicious effect on tumors at the irradiation site and can play a role in rapid tumor shrinkage when GC is combined with cardiac and pyloric obstruction. Furthermore, because of its ability to activate the immune system, it still has a specific effect on systemic metastatic lesions, and the adverse reactions are mild. In this Review, we provide an overview of the current application progress of PDT for GC; systematically elaborate on its principle, mechanism, and the application of a new photosensitizer in GC; and focus on the efficacy of PDT in GC and the prospect of combined use with other therapeutic methods to provide a theoretical basis for clinical application.
