Pial arteriovenous fistula: a review of literature

Abstract: Pial arteriovenous fistula (AVF) is a rare vascular lesion, with less than 120 reported cases in the English literatures (Hoh et al., Neurosurgery 2001;49(6):1351). The angio-architecture, clinical course and therapeutic options are all different from arteriovenous malformation (AVM), dural AVM or other intracranial vascular lesions. A review of literatures to analyse the clinical course of pial AVF was carried out. The presence of varix dictates the clinical course and presentation. Paediatric type had high p… Show more

“…53 In our literature review, the mean age of patients presenting with ICH was 6.4 ± 5.5 years versus a Table 3). Of the 108 patients who underwent embolization as their initial treatment and had data reported on fistula obliteration, 74 (68.5%) had angiographic evidence of complete obliteration of their PAVF.…”

“…18,48,53 They are typically classified as a subtype of pial AVMs, yet are differentiated by their lack of an intervening nidus between the arterial feeder(s) and venous drainage. 24 Pial AVFs have been found to account for 4.7% of pial AVMs; 47 however, they tend to be overrepresented in the pediatric population at an increased incidence of 17.2% of pial AVMs.…”

“…20 Weon et al reported that in 34 of 35 pial AVFs treated by endovascular methods, 2 to 3 sessions were required in nearly half of the cases, and in 17% of the cases, 4 to 5 sessions were necessary. They pointed out that the arterial feeding vessels are often tiny and difficult to access, and the cumulative risk of multiple, staged embolization procedures may exceed the risk of craniotomy for superficial lesions.…”

“…Large numbers of embolic agents such as balloons, coils, glue, Onyx and NCBA have been used for the successful occlusion of the fistula (9,15). However, there was a review reported by Yang et al (13), surgery had a higher obliteration rate (96.8%) than endovascular embolization (86.5%). However, we should pay more attention to the indication for surgery was only for selected superficial fistulas.…”
