Pigmented atypical fibroxanthoma

Abstract: Pigmented atypical fibroxanthoma represents a poorly recognized variant of the neoplasm that may be easily misinterpreted as malignant melanoma. To the best of our knowledge, this is the first report of erythrophagocytosis in atypical fibroxanthoma.

“…25,26 In addition, pigmented variants of AFX mimicking melanoma have been reported in the literature. 27 LMSs are most frequently found in young adults presenting as painful subcutaneous nodules on the extremities. Superficial cutaneous LMSa are rarely encountered spindle cell tumors of sun-damaged skin.…”

“…Histologically, these lesions overlap with benign lesions such as common or cellular blue nevi, desmoplastic nevi, and pigmented neurofibromas and with malignant lesions such as pigmented atypical fibroxanthoma, melanoma, and Bednar tumors, if they have some atypical cells. 14,15 In the vast majority of nevi and melanoma, the nonpigmented lesional cells express S100 and/or specific melanocytic markers, whereas pigmented fibrous proliferations, including dermatofibromas and DFSPs, would be negative for these markers. Although pigmented neurofibromas can express CD34, like other neural lesions, this possibility can be excluded from the differential diagnosis by a negative S100.…”
