Pimozide Treatment in Disruptive Severely Retarded Patients

Abstract: Eight severely retarded residents took part in a double blind, placebo controlled trial of pimozide. The drug did not alter behavioural adjustment as assessed by behaviour observations. However, ratings on a standardised checklist of aberrant behaviour showed significant drug-induced improvements on two dimensions (Irritability and Hyperactivity/non-compliance). Learning performance, as assessed by a discrimination learning task, was not affected by the drug.

“…The authors' report of internal consistency is high (mean subscale alpha coefficient of .90), inter-rater reliability is moderate to high (mean r l .63), and test-retest reliability ranges from .96 to .99 for each of the subscales (Aman, Singh, Stewart, & Field, 1985). Criterion group validity and the validity of the instrument in relation to other methods of assessing behaviour are also good (Aman & Singh, 1986) and the ABC has proved to be a sensitive measure of drug effects, including effects of stimulant and antipsychotic drugs (Aman & Singh, 1982 ;White & Aman, 1985).…”

“…Three studies included only adults with intellectual disabilities: a double-blind placebo controlled crossover trial 32 and a single-case experimental reversal design of pharmacotherapy, 33 as well as a single-case experimental reversal design of a psychological intervention. 34 Two studies included children and young people: a randomised trial of pharmacotherapy by White and Aman 35 and a single-case study of a psychological intervention for a 13-year-old girl. 36 …”

“…White and Aman 35 evaluated the use of pimozide on maladaptive behaviours and hyperactivity, in young people and adults with moderate to profound intellectual disabilities. Following a 4-week baseline, the eight participants received two 4-week treatments with either pimozide, in a dosage of 0.12 mg/kg/day, or placebo, with a 1-week washout period between intervention phases.…”
