Pineal ganglioglioma with premature thelarche

Abstract: We report a case of a 6-year-old girl with a ganglioglioma in the pineal region presenting with a rare clinical picture of premature thelarche. Intracranial gangliogliomas are rare, especially in the pineal region. Their character, origin, treatment, and prognosis remain controversial. The pathophysiology of precocious puberty associated with pineal ganglioglioma is discussed.

“…The biology of the tumor is determined by the glial component, in which neoplastic changes can occur. Most of the gangliogliomas are found in the temporal lobe, but can also be seen in other regions of the central nervous system: third ventricle, hypothalamus (Johannson et al, 1981), optic nerve (Sadun et al, 1996;Liu et al, 1996), pineal region (Chang et al, 1996), and cerebellum (Handa et al, 1994). Immunoreactivity for neuronal antigens, like neuron specific enolase, synaptophysin or neurofilament in the neuronal component can be detected as well as immunoreactivity for guai antigens like guai fibrillary acid protein or S-100 protein.…”

“…Our review of the literature on pineal tumors indicates that sleep difficulties are rarely, if ever, considered to be part of the clinical problems. [13][14][15] We wish to emphasize the importance of obtaining a sleep history in patients with pineal lesions and the need for melatonin levels and replacement therapy in those who have chronic sleep difficulties.…”
