Placenta percreta – an obstetrician's nightmare

Abstract: Placenta percreta is a rare but a life threatening condition. Control of massive haemorrhage is the first priority; however, the patient's desire for future fertility has to be taken into consideration. Nevertheless, in cases of massive haemorrhage, hysterectomy should be carried out without delay to prevent major complications or even maternal death. Here we present a case where we had to do a quick subtotal hysterectomy because of torrential bleed due to placenta percreta.

“…13 In a case report of a case of placenta percreta by Mohan P et al had subtotal obstetric hysterectomy done for the case of placenta percreta and intraoperative blood loss was approximately 3.3 litres which was replaced with transfusion of 8 units of whole blood, 3 units of fresh frozen plasma and 2 units of platelet concentrates during the per-operative and postoperative period. 14 Morbid adhesion of placenta should be suspected even in women without any risk factors when there is postpartum hemorrhage and the same should be ruled by performing an immediate Ultrasonogram.…”

“…Patients with PAS disorders are associated with an increased rate of maternal morbidity and mortality, including massive blood transfusion, cystotomy, postpartum infection, peripartum hysterectomy, prolonged hospital stay, intensive care unit (ICU) admission, and even death [ 5 8 ]. PAS disorders are significant life-threatening issues due to the increased incidence, morbidity, and mortality, and it is called “obstetrician's nightmare” among obstetricians [ 9 , 10 ].…”

“…Placenta accreta (PA) is an obstetrician's nightmare [1] because of massive life threatening hemorrhage and cesarean hysterectomy (CH) is the only recommended resort in such circumstances to save a woman's life. The incidence of placenta accrete is rising rapidly because of rising cesarean delivery rate.…”
