Plasma Cortisol in Sickle Cell Disease

Abstract: The plasma cortisol levels of 108 children aged 1–16 years with various Hb genotypes (AA, AS and SS) were measured. The mean plasma cortisol levels of both the AS and SS groups (during steady state) fell within the normal range but the mean of the SS group is significantly lower than the AA group. In fact, 9 of the sickle cell children had plasma cortisol levels below the normal range while no child in the HbAS and HbAA groups had plasma cortisol below the normal range. These results indicate that in sickle ce… Show more

“…In this study, our observation was similar to that of Osifo et al [7] as we observed that cortisol level was lower in SCA patients but, increased significantly during painful crisis toward the level in control subjects. This is similar to the observed elevated level of CRP in the VOC group compared with the SCA subjects in steady state.…”

“…They observed that plasma cortisol concentrations were diminished during painful crises. Osifo et al [7] reported that cortisol production is lower in SCA patients but, increases during painful crisis. In contrast, Saad and Saad [41] infused synthetic adrenocorticotropic hormone in SCA subjects in steady state and observed that the subjects had similar level of cortisol compared with controls.…”

“…In one report by Osifo et al, the mean plasma cortisol level in HbSS steady-state SCA patients was significantly lower than patients with no SCA; however, only 8% of SCA patients had plasma cortisol below the normal range 22. In another report, basal plasma cortisol levels were lower in children with the HbSS genotype compared with those of other genotypes 4.…”

“…Plasma triiodothyronine and thyroxine concentrations were normal in children and adolescents with HbSS (31,35), but plasma triiodothyronine concentrations were decreased in adult men with HbSS (40). Plasma cortisol concentrations were significantly lower in children with HbSS than in control children (39). Plasma concentrations of growth hormone were lower in adolescents but greater in young adults with HbSS compared with controls (37).…”
