Plasma N‐terminal pro BNP and cardiotrophin‐1 are elevated in aortic stenosis

Abstract: Background: Echocardiography with Doppler examination of the aortic valve provides a very accurate assessment of the Ž . Ž . transvalvular gradient and is used to monitor progression of aortic stenosis AS . Plasma brain natriuretic peptide BNP has been shown to correlate with end-systolic wall stress in patients with AS. Aim: We hypothesized that plasma N-terminal Ž . Ž . proBNP NT proBNP and a newly identified cytokine cardiotrophin-1 CT-1 , which has been shown to stimulate BNP production at a transcriptiona… Show more

“…Increased wall stress and myocardial stretch have been shown to release BNP, which has been referred to as the only "true" ventricular hormone, in various cardiac entities 26 -28 and also in previous studies of AS. 14,16 Patients with TS AS had indeed higher gradients, whereas EF was similar in both groups. Because BNP increases with the severity of LV damage of any cause, it is not surprising that the overlap of BNP levels between PS AS and TS AS was too extensive to allow their differentiation in individual patients.…”

“…These results suggest NT-proBNP and CT-1 could potentially be used to monitor progression of disease non-invasively. These markers may also be useful to identify the optimum time for surgery in AS 47 .…”

“…1 Our group has described that CT-1 induces cell proliferation, hypertrophy, and secretion of extracellular matrix proteins in aortic vascular smooth muscle cells (VSMCs) in vitro 2 and in vivo, 3 suggesting a role for CT-1 in arterial fibrosis and stiffness, which both are features of aging. Furthermore, elevated plasma concentrations of CT-1 have been reported in patients with a variety of diseases, including hypertension, 4 heart failure, 5 uns table angina, 6 myocardial infarction, 7 and aortic stenosis, 8 indicating that this cytokine may contribute to the development of cardiovascular pathologies associated with aging.…”
