Plasma Propranolol Levels Associated with Suppression of Ventricular Ectopic Beats

Abstract: SummaryThe plasma propranolol levels associated with the abolition of chronic, stable ventricular ectopic beats have been studied. Racemic propranolol (DL) suppressed the ectopic foci at plasma levels of 40-85 ng/ml in eight patients, but levels of 70-200 ng/ml were unsuccessful in four patients. Dextro-(D) propranolol levels of 180-310 ng/ml were ineffective in four patients who had previously responded at levels of 60-75 ng/ml of racemic propranolol. The significance of the sympathetic nervous system in the … Show more

“…Both these properties are included in the term 'membrane stabilizing activity', and there has been a great deal of controversy about its contribution to the effects of P-adrenoceptor blocking drugs in angina pectoris and in cardiac arrhythmias in man. However, the membrane stabilizing effect of propranolol has been demonstrated in vitro with human muscle only at a minimum concentration of 10 mg/l (Coltart & Meldrum, 1970) which is about 100 times greater than that required for inhibition of exercise tachycardia or suppression of ectopic beats (Coltart & Shand, 1970;Coltart, Gibson & Shand, 1971). This is, therefore, not thought to be an important property of,B-adrenoceptor blocking drugs in the doses used in clinical practice and there are no in vivo methods for demonstrating it in man.…”

“…At each visit, patients were examined and a plasma sample was obtained [5][6] hours after a dose of either placebo or active drug. A 24-hour ambulatory ECG recording was obtained each week using Medcraft Accutape Holter-type recorders.…”
