Plasma renin substrate sensitivity to oestrogens and oestrogen metabolism in cirrhosis

Abstract: Oestrogen stimulation of plasma renin substrate (PRS) was studied in men with alcoholic cirrhosis. PRS values, before and 1, 2, 4 and 6 days after a single oral administration of 100 microgram of an oestrogen derivative, 11beta-methoxy-17-ethynyl-1,3,5(10)-estratriene-3,17beta-diol (Moxestrol), were measured by radioimmunoassay of generated angiotensin I in five men with normal liver function and five men with alcoholic cirrhosis. Basal PRS was 0.93 +/- 0.22 nmol/ml (mean +/- 1 SD) in the normal men and signif… Show more

“…A rise of PRS requires a de novo synthesis of this hepatic protein (Nasjletti & Masson 1972). The time intervals between the hormone injection and the changes of PRS agree well with earlier findings on oestrogen-induced protein synthesis in man (Degos et al 1978) and in animals (Jackson 1973;Luskeyetal. 1974).…”

“…In interpreting the PRA, the clinician needs to be aware of not only conditions that affect the renin concentration but also those that affect the angiotensinogen concentration. Plasma angiotensinogen concentrations are increased during pregnancy, glucocorticoid excess, and estrogen administration (22, 28 -30 ) and are decreased when angiotensinogen production is reduced in liver disease (21,30 ). Renin secretion is subject to tonic negative-feedback inhibition by Ang II (Fig.…”

“…Because the "plasma renin activity" changes with the plasma content of Substrate for renin, values for "plasma renin activity" and "plasma renin concentration" (measured with excess of exogeneous renin Substrate) show discrepancies in some conditiöns such äs pregnancy, liver cirrhosis and high renin hypertension (5,6,10). In the present study, the correlation between both enzynaatic methods was abnormal in samples with a high renin content (plasma renin measured with endogeneöus Substrate ^10 g/l · h) and no correlation was observed in those with a low renin conterit (plasma renin measured with endogeneoüs Substratê l g/l · h).…”
