Abstract. Plasma renin concentrations (PRC), plasma renin substrate concentration (PRS), plasma aldosterone and cortisol concentrations as well as plama renin activity (PRA) were measured in ovariectomized subjects after im administration of 10 mg oestradiol benzoate (EB). The esterified oestrogen exerts two independent effects on the renin-angiotensin-aldosterone system. 1) 48 h after EB administration, PRS was significantly increased. Similar results were obtained for total plasma cortisol, reflecting transcortin concentration. In both cases, the increase was quantitatively related to the basal concentrations. These observations are consistent with the well known oestrogen-induced protein synthesis in the liver. 2) The elevation of PRC preceded that of PRS and was already significant 11 h after EB injection. The early rise in plasma renin activity was essentially caused by the increase in PRC, whereas an influence of the activated substrate synthesis was found later, between the 2nd and the 4th day post injection. The time course of plasma aldosterone concentration correlated well with the increased PRA. The results provide evidence that EB has two different effects on the renin-aldosterone axis: an early one by elevating renin release and a delayed one by increasing renin substrate synthesis. Whereas the second mechanism can clearly be localized in the liver, extrarenal as well as direct renal effects of EB may be responsible for the renin stimulation.

BACKGROUND:Measurement of plasma renin is important for the clinical assessment of hypertensive patients. The most common methods for measuring plasma renin are the plasma renin activity (PRA) assay and the renin immunoassay. The clinical application of renin inhibitor therapy has thrown into focus the differences in information provided by activity assays and immunoassays for renin and prorenin measurement and has drawn attention to the need for precautions to ensure their accurate measurement.CONTENT: Renin activity assays and immunoassays provide related but different information. Whereas activity assays measure only active renin, immunoassays measure both active and inhibited renin. Particular care must be taken in the collection and processing of blood samples and in the performance of these assays to avoid errors in renin measurement. Both activity assays and immunoassays are susceptible to renin overestimation due to prorenin activation. In addition, activity assays performed with peptidase inhibitors may overestimate the degree of inhibition of PRA by renin inhibitor therapy. The values for "plasma renin activity", "plasma renin concentration" (both determined by the rate of angiotensin I generation) 2 ) and the plasma active renin protein mass concentration (active enzyme protein measured by a direct radioimmunoassay) were compared. The values for "plasma renin activity" (angiotensin I generation from endogeneous renin Substrate) and "plasma renin concentration" (angiotensin I generation from exogeneous excess of renin Substrate) showed a significant correlation. However, the correlation was lost in samples with high (plasma renin activity ^10 g/l · h) and low (plasma renin activity ^ l g/l · h) renin contents. In another study, values for "plasma renin activity" were correlated with those for plasma active renin protein mass concentration, and values for "plasma renin concentration" were also correlated to those for plasma active renin protein mass concentration. However, in samples with low renin contents (plasma active renin protein mass concentration ^ 20 ng/1), there was nö correlation between values for "plasma renin activity" and plasma active renin protein mass concentration, although those for "plasma renin concentration" and plasma active renin protein mass concentration showed a significant correlation. These results suggest that the current method for the assay of plasma renin activity gives erf oneous values for low and high plasma renin concentrations. The new radiometric method for the determination of the plasma active renin protein mass concentration seems to be applicable to plasma samples with a wide ränge of renin concentrations.
