Plate Fixation of Talus Fractures: Where, When, and How?

Abstract: Talus fractures are rare orthopedic injuries. Surgical fixation is challenging for treating surgeons. The clear majority of fractures require operative treatment. The indication and use of plates in fixation of talus fractures are reviewed. Specific applications including fractures of the lateral process, posterior fractures, extreme comminution, bone grafting, and spring plating to hold key segments are reviewed in this article.

“…Placement of low-profile plates along the flexor hallucis longus tendon groove strikes a good balance between the need for fracture control and tendon irritation. Further studies with longer follow-ups are required to prove the effectiveness and specific long-term results and complications of new plate designs [7,56,[63][64][65][66].…”

“…Screws tend to fail in bending or by pullout, while plates break at their margins. It is thought that smaller more flexible plates might circumvent the issue in the future [63,[67][68][69].…”

“…Fracture reduction and stability, with minimal insult to the bone vascularity, are of paramount importance. The changing thought process toward fixing these by plates to achieve stability is well described by Swords et al ., 10 and the indications and methods are well reviewed. The unusual posteromedial body fractures are also discussed in a separate article, and their association with subtalar dislocations is emphasized, along with the fact that some may be open injuries.…”
