Pleomorphic adenoma‐like tumour of the breast

Abstract: This lesion shows an overlap of morphology and immunophenotype with SG-PA and with MBC. The majority of mammary PAs have a benign behaviour, but local recurrence and development of carcinoma occur. We propose a new terminology of pleomorphic adenoma-like tumour of the breast to reflect the uncertain nature of these tumours and help guide management decisions.

“…The diagnosis of breast PA can be difficult on core biopsy. We and others have reported breast PA as matrix‐producing metaplastic carcinoma on pre‐operative core biopsy that was followed by a benign diagnosis following surgical excision. To avoid such inconsistency, and in view of the above as well as the lack of molecular evidence to help characterize these lesions as benign or malignant, we propose to consider them as lesions of uncertain malignant potential.…”

“…Lymphovascular invasion and distant metastasis have been reported in histologically benign salivary gland PA. The absence of peripheral myoepithelial cells is a feature of breast PA. We believe that the lack of cytological atypia, mitotic activity and focal preservation of peripheral myoepithelial cells represent features of indolence as seen in other low‐grade malignant tumours akin to low‐grade adenoid cystic carcinoma and low‐grade matrix‐producing metaplastic breast carcinoma (MBC), rather than features defining the benign biological nature of a breast tumour . Moreover, circumscription is not always a feature of benign tumours.…”

“…On pathological examination, PA appears as a circumscribed lesion that is clearly demarcated from the surrounding tissue, and is characterized by a mixture of epithelial and mesenchymal components such as glandular ducts, myoepithelial cells, myxomatous stroma, and cartilaginous components. PA diagnosis can be difficult in core biopsy specimens because it must be differentiated from complex fibroadenoma or phyllodes tumor 1,3,4,15 . In addition, two case reports have described misdiagnoses of breast PA identified as matrix-producing metaplastic breast cancer in core-needle biopsy specimens 4,15 .…”

“…Pleomorphic adenoma of breast (PAB) is a rare mammary tumor of a mixed epithelial-myoepithelial nature and is part of the so-called salivary gland-type mammary tumors, initially recognized in 1906. The existence of such tumors in the breast can be attributed to a shared embryonic origin between salivary and mammary glands (1). Although most patients are female with a wide age range, a few cases have also been reported in males.…”
