Pluripotent Stem Cell Models of Human Heart Disease

Jervell and Lange-Nielsen syndrome (JLNS) is one of the most severe life-threatening cardiac arrhythmias. Patients display delayed cardiac repolarization, associated high risk of sudden death due to ventricular tachycardia, and congenital bilateral deafness. In contrast to the autosomal dominant forms of long QT syndrome, JLNS is a recessive trait, resulting from homozygous (or compound heterozygous) mutations in KCNQ1 or KCNE1. These genes encode the α and β subunits, respectively, of the ion channel conducting the slow component of the delayed rectifier K + current, I Ks . We used complementary approaches, reprogramming patient cells and genetic engineering, to generate human induced pluripotent stem cell (hiPSC) models of JLNS, covering splice site (c.478-2A>T) and missense (c.1781G>A) mutations, the two major classes of JLNS-causing defects in KCNQ1. Electrophysiological comparison of hiPSC-derived cardiomyocytes (CMs) from homozygous JLNS, heterozygous, and wild-type lines recapitulated the typical and severe features of JLNS, including pronounced action and field potential prolongation and severe reduction or absence of I Ks . We show that this phenotype had distinct underlying molecular mechanisms in the two sets of cell lines: the previously unidentified c.478-2A>T mutation was amorphic and gave rise to a strictly recessive phenotype in JLNS-CMs, whereas the missense c.1781G>A lesion caused a gene dosage-dependent channel reduction at the cell membrane. Moreover, adrenergic stimulation caused action potential prolongation specifically in JLNS-CMs. Furthermore, sensitivity to proarrhythmic drugs was strongly enhanced in JLNSCMs but could be pharmacologically corrected. Deciphering the fundamental mechanisms controlling cardiac specification is critical for our understanding of how heart formation is initiated during embryonic development and for applying stem cell biology to regenerative medicine and disease modeling. Using systematic and unbiased functional screening approaches, we discovered that the Id family of helix-loop-helix proteins is both necessary and sufficient to direct cardiac mesoderm formation in frog embryos and human embryonic stem cells. Mechanistically, Id proteins specify cardiac cell fate by repressing two inhibitors of cardiogenic mesoderm formation-Tcf3 and Foxa2-and activating inducers Evx1, Grrp1, and Mesp1. Most importantly, CRISPR/Cas9-mediated ablation of the entire Id (Id1-4) family in mouse embryos leads to failure of anterior cardiac progenitor specification and the development of heartless embryos. Stofnfrumur hafa tvenns konar eiginleika: við frumuskiptingu geta þaer annaðhvort endurnýjað sig eða sérhaefst í aðra frumugerð. Stofnfrumur úr fósturvís-um (ES-frumur, embryonic stem cells) eru einangraðar úr fósturvísum. Þaer eru fengnar úr glasafrjóvgunum með upplýstu samþykki aðstandenda og þeim hefði annars átt að farga.1 Þaer teljast fjölhaefar (pluripotent) því þaer geta sérhaefst í hvaða frumugerð líkamans sem er, ólíkt vefjasértaekum stofnfrumum (tissue specific stem cells) sem geta aðeins sérhaefst í tiltekna frumugerð í vefnum sem þaer finnast í, og eru því kallaðar marghaefar (multipotent).2 Í gegnum tíðina hafa stofnfrumur úr fósturvísum verið ranglega nefndar fóstur-stofnfrumur. Í fóstri eru engar fjölhaefar stofnfrumur heldur aðeins vefjasértaekar stofnfrumur. Margir renna hýru auga til ES-frumna sem úrraeðis til laekninga á ýmsum sjúkdómum, þar sem haegt verður að sérhaefa þaer í ákveðnar frumugerðir sem sjúklingar þurfa á að halda, og vaeri þá haegt að koma þeim frumum fyrir í sjúklingnum. Sá böggull fylgir þó skammrifi að vefjaflokka prótein eru tjáð í ES-frumunum og gaeti það orsakað höfnun frumnanna við ígraeðslu. Fósturþroskun mannsinsOkfruma myndast við samruna eggs og sáðfrumu. Hún skiptir sér á fyrstu stigum fósturþroska og verður að hneppifóstri (morula) á 16. frumustigi. Kímblaðran (blastocyst) myndast svo á 4-5 degi og innheldur hún innri frumumassa (inner cell mass) umlukinn ytra lagi af naeringarhýði (trophectoderm) sem síðar þroskast í utanfósturvefi, svo sem fylgjuna. Innri frumumassinn Stofnfrumur úr fósturvísum eru einangraðar úr fósturvísum eins og nafnið bendir til. Þetta eru fjölhaefar frumur sem geta annaðhvort endurnýjast og haldist ósérhaefðar eða sérhaefst í hvaða frumugerð sem er í líkamanum. Árið 1998 tókst að einangra stofnfrumur úr fósturvísum manna og breytti það sýn manna á nýja möguleika í vefjalaeknisfraeði. Aðeins 8 árum síðar tókst vísindafólki að mynda svokallaðar iPS-frumur, fjölhaefar stofnfrumur sem útbúnar voru með því að endurforrita líkamsfrumur. Þetta hefur gjör-bylt hugmyndum um óafturkraefi frumuþroska. Í kjölfarið hefur mikið verk verið unnið til þess að kryfja til mergjar sameindalíffraeði fjölhaefra stofnfrumna. Unnt er að mynda iPS-frumur úr líkamsfrumum sjúklinga og hafa þaer því sama genamengi. Þessar frumur eru því einstaklega nytsamar á ýmsum sviðum laeknisfraeðinnar og má meðal annars nýta þaer til að skilja sjúkdómsframvindu, framkvaema lyfjaprófanir og vefjaígraeðslur.ÁgrIp myndar kímþekju (epiblast) sem við myndun holfósturs (gastrulation) á 14. degi þroskast í fósturlögin þrjú, innlag, miðlag og útlag. Eitt af aðalsmerkjum holfóstursmynd-unar er myndun frumrákarinnar og sérhaefing hinna þriggja fósturlaga.1 Fjölgun og frumuskrið kímþekju-fruma fer fram í ferli sem kallast bandvefsumbreyting þekjuvefjar (Epithelialtomesenchymal transition, EMT) en þá minnkar viðloðun milli frumna og tenginga þeirra við grunnhimnu og frumuskrið á sér stað.
