“…Postulated causes of pneumocele include iatrogenic, developmental (arachnoid cyst), inflammatory, neoplastic (optic nerve sheath meningioma, fibro-osseous disease), and traumatic, leading to obliteration of the sinus and subsequent hyperinflation. 8 , 9 Wolfensberger and Herrmann were the first to document the trap valve hypothesis in a patient with a maxillary sinus pneumocele. 10 The theory is that redundant mucosa or a stricture may create a one-way valve between the nasal cavity and the affected sinus, and produce chronic air trapping, intrasinus pressure disequilibrium, and hyperpneumatization of the affected sinus.…”

“…They have been described as primary or secondary. The proposed aetiologies of primary cases include a one-way valve effect,8 gas-forming organisms,10 spontaneously discharged mucocele,11 trauma12 and dysregulation of hormones. However, there is no report of any hormonal alteration so far.…”

“…This is not always the case in other similar cases of sinus expansion. Complications of pneumosinus include cosmetic deformities due to the bossing of the facial bones [11], recurrent sinusitis due to obstruction of the paranasal sinus outflow tract [6], and proptosis, diplopia, or progressive blindness due to expansion towards the orbit [12]. We believe that in similar rare cases of abnormal sinus or mastoid air cells expansion, the treatment should be individualized and address the patients cardinal symptoms.…”

“…We believe that in similar rare cases of abnormal sinus or mastoid air cells expansion, the treatment should be individualized and address the patients cardinal symptoms. An algorithm on surgical management of pneumosinus dilatans of the frontal sinus has been described by Patel et al [11] but can be generalized for all aerated cavities. If the main symptoms are caused by obstruction of the sinuses ostia, Functional Endoscopic Sinus Surgery (FESS) may restore normal drainage.…”
