Polyendocrine autoimmunity

Abstract: Autoimmune polyendocrine disorders are characterized by the presence of more than one autoimmune endocrine disease. Study of such disorders has provided unique insight into the pathogenesis of autoimmunity and the development of tolerance in the normal individual. Autoimmune polyendocrine syndromes I and II have distinctive inheritance patterns, genetic causes, component diseases, and implications for follow-up. Care for individuals with these diseases requires monitoring for additional autoimmune disorders an… Show more

“…As a whole, these data suggest a possible immune system dysregulation in MD patients; if this is true, a predisposition to autoimmunity and an association with other autoimmune diseases should be observed. In this setting, thyroid gland represents a useful paradigm to be studied, given the high number of autoimmune diseases to which Hashimoto's thyroiditis has been associated [Barker, 2005]. An association between AITD and MD has been postulated for over 30 years, and was recently confirmed by our and other studies [Brenner et al, 2004;Fattori et al, 2008].…”

“…Indeed, autoimmune diseases are often associated with one another, producing a polyendocrine syndrome in which alterations in the immune system involving more than one gland generally cause hypofunctioning conditions. Multiple gland autoimmune (MGA) syndromes include a group of diseases featuring the association of one or more endocrine disorders and often of autoimmune diseases that do not involve endocrine organs [30]. Three principle types of MGA on the grounds of the involved glands and organs have been described.…”

“…Fattori et al [20] further discussed that pendrin, associated with Pendred syndrome, may be a target for an autoantigen as it is expressed in both the thyroid gland and inner ear. Additionally, Fattori et al [20] suggested that autoantibodies in MD may predispose patients to other autoimmune diseases and manifest clinically as polyendocrine syndrome including multiple gland autoimmune syndromes [30]. For example, Hashimoto’s thyroiditis has been seen in multiple gland autoimmune syndromes such as systemic lupus erythematosus, pernicious anemia, and rheumatoid arthritis [31, 32].…”
