Polymerase chain reaction (PCR) detection of B cell clonality in Sjögren's syndrome patients: a diagnostic tool of clonal expansion

Abstract: SummarySjögren's syndrome (SS) is an autoimmune disease characterized by clonal B cell attack of the exocrine glands and dysregulated expression of B cellactivating factor (BAFF). Based upon the current data of increased rates of lymphoid malignancy, as non-Hodgkin's lymphoma (NHL) is associated with SS, we propose the detection of clonal rearrangements of immunoglobulin heavy chain (IgH) gene in those patients as a predictor of malignant clonal expansion. To test our proposal, we examined the IgH clonal rearr… Show more

“…To evaluate whether the time of cART initiation might influence the mucosal B cell repertoire in HIV-1-infected individuals, we studied B cell clonality in the e-ART and l-ART groups (26). All patients in both groups displayed a normally distributed polyclonal profile (Fig.…”

“…We would like to consider as one of the examples, the case where monoclonal B-cell proliferation is accompanied by chronic inflammation. Monoclonal B-cell growth in chronic inflammation has been reported in Hashimoto's thyroiditis, 8 chronic hepatitis C, 9 Sjögren's syndrome, 10 and Helicobacter pylori-induced chronic active gastritis. 11 Fan et al 9 supposed the monoclonality to be a sign of future progression to malignancy.…”

“…Guzman et al [67] determined whether a clonal B-cell population was present in the minor salivary glands of patients with Sjogren's syndrome (n=60) and healthy controls [42]. They found a clonal population in 87% and 19% respectively, indicating indeed that reactive lesions may be clonal when a small B-cell population is present and that in autoimmune diseases a high percentage can be found due to the aberrant B-cell regulation.…”
