Poor tolerability of cystic fibrosis transmembrane conductance regulator modulator therapy in lung transplant recipients

Abstract: Elexacaftor/tezacaftor/ivacaftor (ELX/TEZ/IVA) is a highly effective therapy for patients with cystic fibrosis (CF) with potential benefits in lung transplant recipients (LTRs) for extrapulmonary CF manifestations such as sinus disease, gastrointestinal disease, weight loss, and pancreatic insufficiency. Conversely, when ELX/TEZ/IVA was prescribed to lung transplant recipients for extrapulmonary complications of CF, triple therapy was poorly tolerated with modest perceived extrapulmonary benefit so that about 40% of patients discontinued the medication ( Doligalski et al, 2022 ; Ramos et al, 2022 ).
