Population Pharmacokinetic Investigation for Optimization of Amiodarone Therapy in Japanese Patients

Abstract: The authors developed new population pharmacokinetic parameters. Clinical application of the findings in the present study to patient care may permit selection of an appropriate initial maintenance dose, thus enabling the clinician to achieve a desired therapeutic effect.

“…In the current study, male patients had an increased risk of cancer, but female patients did not. One possible explanation for this difference is that there is a 37% higher clearance rate of amiodarone in females than in males because of differences in cytochrome P450 3A4 activity and the percentage of body fat 25. In addition, hormonal stimuli might play a role in the gender difference.…”

“…22 Amiodarone is metabolized by cytochrome P450 (CYP) 3A4 and CYP2C8 in the liver, and amiodarone and its metabolite are both highly lipophilic and may accumulate in many tissues, especially adipose tissue. 23 A pharmacokinetic investigation in Japan revealed 37% faster amiodarone clearance in females compared with males. The proposed mechanism related to the impacts of estrogen and progesterone on CYP3A4, higher CYP3A4 activity in hepatic microsomes of females, and lower body fat in females.…”

“…Females have increased levels of cytochrome P450 3A4, the most abundant phase I drug-metabolizing enzyme in humans, as well as an increased percentage of body fat (10% -13% versus 2% -5% in males). These may work in combination to increase the clearance rate of amiodarone ~37% [43]. Differences in hormonal stimuli may also play a role.…”
