Position paper of the Italian Society for the study of Dementias (Sindem) on the proposal of a new Lexicon on Alzheimer disease

Abstract: A panel of Italian neurologists of the Italian Society for the study of Dementias (SINDEM) discussed the recently proposed new lexicon for Alzheimer disease (AD) and the related diagnostic criteria for the different phases of the disease (Preclinical AD, prodromal AD and Alzheimer's dementia) (Dubois et al. in Lancet Neurol 6:734-746, 2007; in Lancet Neurol 9:1118-1127, 2010). The aim of this discussion was to reach a consensus, among the Italian neurologists involved in the study and care of persons with deme… Show more

“…The new criteria proposed a common lexicon for the clinical and research sectors [12], which has generated much debate in the scientific community [14][15][16]. Developments in the field of biomarkers and the use, alongside traditional genetics, of new more specific and more sensitive techniques were crucial for the development of these new criteria, but also for redefining frontotemporal dementia (FTD) and its subtypes.…”

“…Obviously, this proposal facilitated the AD trials and has been accepted by many investigators. Otherwise, numerous critiques have emerged regarding the new biological criteria for AD [268,269], some of them being severe because they affected the methodology and its ethical aspects [138,[269][270][271][272][273]. Therefore, in this very critical context of biological and clinical breakaway, the relevant modification of the new conceptualization of AD presented by Dubois et al, in 2021 [274], with the inclusion of clinical dementia in its definition is comprehensible.…”

“…The results were summarised in two series of recommendation papers, one from the International Working Group for New Research Criteria for the Diagnosis of AD (IWG), [2][3][4] the other, from the National Institute on Aging-Alzheimer's Association (NIA-AA). [5][6][7] Later, the Italian Society for the study of Dementias (SINDEM) 8 adhered to part of the IWG positions.…”
