Possible Gatifloxacin-Induced Fulminant Hepatic Failure

Abstract: Fluoroquinolones, including trovafloxacin, ciprofloxacin, ofloxacin, enoxacin, norfloxacin, and, in this case report, gatifloxacin, have been associated with hepatotoxicity. It is important that these medications be considered a possible cause when the patient being treated has liver disease.

“…Severe hepatocellular injury and cholestasis have been reported to occur in less than 1% of all fluoroquinolone recipients, excluding trovafloxacin, which was withdrawn due to its hepatotoxi-city (4,(134)(135)(136)(137). Clinically significant hepatotoxicity has been reported with ciprofloxacin, trovafloxacin, norfloxacin, ofloxacin, enoxacin, levofloxacin, and gatifloxacin, with large population denominators (138,139). Direct comparisons of rates of clinically significant hepatotoxicity are not available.…”

“…60 Isolated cases of significant hepatotoxicity have been reported with ciprofloxacin, 61 levofloxacin, 62 gatifloxacin. 63,64 The mechanism of hepatotoxicity is thought to be due to hypersensitivity reaction often associated with peripheral eosinophilia and fever. Introduction of fluoroquinolones didn't cause additional hepatotoxicity when used in patients with hepatitis induced by first line anti-TB drugs.…”

“…5,32 This is frequently observed with amoxicillin/clavulanate and tetracycline, 32 and was reported for trovafloxacin, 33 although rapid onset is also seen. 33,34 Cholelithiasis associated with ceftriaxone–calcium precipitates typically occurs only after 9–11 days of treatment. 32 Physicians should therefore alert patients to the warning signs of antibiotic-induced hepatotoxicity and advise them to seek medical attention, even if symptoms appear sometime after completing their treatment.…”
