Post-infectious neurological disorders

Abstract: A multitude of environmental factors can result in breakdown of immune tolerance in susceptible hosts. Infectious pathogens are among the most important environmental triggers in the pathogenesis of autoimmunity. Certain autoimmune disorders have a strong association with specific infections. Several neurological autoimmune disorders are thought to occur through post-infectious mechanisms. In this review, we discuss the proposed mechanisms underlying pathogen-induced autoimmunity, and highlight the clinical pr… Show more

“…According to Blackburn and Wang ( 72 ), the proposed mechanisms of post-infectious neurological disorders include molecular mimicry, epitope spreading, bystander activation and polyclonal B-cell activation. Molecular mimicry is due to the presence in microorganisms of epitopes that share marked similarity in peptide sequence or three-dimensional structure to host’s antigens.…”

“…Recurrent herpetic infections also appear to play a role in several postinfectious autoimmune neurological disorders. Herpes Simplex virus (HSV) infection can lead to Herpes Simplex Virus encephalitis, which evolves in autoimmune encephalitis in up to 27% of cases, while Varicella Zoster Virus (VZV) has been associated to Neuromyelitis Optica spectrum disorders [30]. Activation of innate immunity is another important mechanism facilitating autoimmunity ( Table 1).…”

“…PINS etiology is unfortunately unknown because of few studies performed. It is assumed that it is the result of a transient autoimmune response toward myelin or other self-antigens through molecular mimicry during the infectious episode that precedes symptoms, leading to destruction of cerebral, spinal cord and optic nerves white matter causing demyelination [ 6 , 7 ].…”
